This Service Agreement (the “Agreement”) is  by and between Skywave Wireless Internet and the Subscriber information and date listed on the front side.

The Parties agree as follows:

1.   Skywave Wireless Internet will provide wireless internet service for a monthly fee.  This service will be available 24 hours a day except during scheduled maintenance or repair.

2.   The services described above shall commence on  the work order completion date and shall be provided for the term of 12 months from that same date.  In the event of cancellation of service before the expiration of the 12 months, there will be a $50 cancellation fee. At the end of 12 months, service will be provided on a month to month basis.

3.  The Subscriber shall pay Skywave Wireless Internet for the material and labor to be performed under the Agreement the sum of  $150.00 for basic installation which includes the mounting of one subscriber module and antenna on the house and 50 feet of Ethernet cable.  Any additional equipment or wire will be at the expense of the Subscriber.  Any additional installation charges will be discussed and approved on the front of this form and initialed by the Subscriber.   This installation fee does not purchase of the installed items.  All equipment and cable remain the property of Skywave Wireless Internet at all times.  The Subscriber is simply being allowed to "lease" the equipment.  At the termination of this contract, a Skywave representative will remove the equipment from the subscribers house.  Any intentional abuse by the subscriber of the Skywave equipment will result in the subscriber being responsible for the replacement cost of the equipment at the minimum rate of $350.00 .

4. The Subscriber shall pay Skywave Wireless Internet the monthly fee associated with the plan chosen on the front of the work order.  This monthly fee is to be paid on or before the 5th of each month and is paid for the upcoming month.. 

Prepayment will receive the following discounts: 

* 3 month pre pay will receive a 3% discount  

*6 month prepay will receive a 5% discount

*12 month prepay will receive a 10% discount.

The payments shall be paid in the following manner:

                         Paypal payments can be sent to Info@SkywaveInternet.com

                         Credit Card Payments can be called in or set up for automatic monthly payments.

All payments must be reached by Skywave on or before the 5th of each month to prevent interruption of service.

5.   (a)  All work shall be completed in a workmanship like manner, and if applicable, in compliance with all building codes and other applicable laws.

(b)  To the extent required by law all work shall be performed by individuals duly licenses and authorized by law to perform the work.

 (c)  Skywave Wireless Internet is not liable for any elemental damage that may be caused by the installed equipment or any structural damage due to storms or acts of nature.

(d)  Skywave Wireless Internet shall not be liable for any delay due to circumstance beyond its control.

(e)  Installers are independent Contractors and not an employee of  Skywave Wireless Internet. .

(f)  Any changes to this document will become binding upon email notification to subscriber.  Any changes will be posted at SkywaveInternet.com.

(g) This is a partial contract and subscriber acknowledges that there may be other rules and regulations governing this agreement.  These regulations may include but are not limited to insurance contracts and liability agreements.
